When home movies are passed from a family or collector to an archival institution, it is crucial to pass along as much information about the materials as possible, so that the images can be understood and contextualized most fully. To this end, this “Donation Questionnaire” was developed by archivists working with amateur film, and may be downloaded and used by film donors or archives alike to record such information, for use by future viewers.
